You will receive a patient satisfaction phone survey call in regards to your stay with Montclair Hospital Medical Center within a couple of weeks of discharge. Please complete the phone survey so that we can maintain the highest standards in our patient services.
Anytime your concerns are not resolved to your satisfaction, you may also contact our Administration Office at (909) 625-8192. They will work with all hospital departments to investigate your complaint and assist you in handling your concern. If this is not to your satisfaction, you may file your complaint with the California Department of Health Services by calling 909-383-4777.
