The American Heart Association presents the Get With The Guidelines® – Heart Failure Gold Plus award for proven dedication to ensuring all heart failure patients have access to best practices and life-saving care
MONTCLAIR, CA, Aug. 25, 2025 — Montclair Hospital Medical Center, member of the Prime Healthcare Foundation, has received the American Heart Association’s Get With The Guidelines® Heart Failure Gold Plus quality achievement award for its commitment to improving outcomes for patients with heart failure, helping reduce readmissions and increase healthy days at home.
Heart failure affects more than 6.7 million adults in the United States, a number projected to grow to over 8 million by 2030. While there is no cure, patients can live well by following a care plan that includes medication, symptom monitoring, and lifestyle changes. Get With The Guidelines® supports hospitals nationwide in delivering the latest evidence-based treatment for heart failure, improving survival and long-term health.

In addition to the Gold Plus award, Montclair Hospital earned placement on the American Heart Association’s Target: Heart Failure Optimal Honor Roll, meeting criteria that improve medication adherence, early follow-up care, and patient education. The hospital also received the Target: Type 2 Diabetes Honor Roll™, recognizing excellence in providing the most up-to-date, evidence-based treatment for patients with Type 2 diabetes hospitalized with heart disease or stroke.
