Fall 2025 Safety Grade recognizes national leadership in patient safety
MONTCLAIR, Calif., November 13, 2025 – Montclair Hospital Medical Center, a member of Prime Healthcare, has earned an “A” Hospital Safety Grade from The Leapfrog Group, an independent national watchdog organization committed to improving healthcare quality and safety. Leapfrog assigns an A, B, C, D, or F to general hospitals nationwide based on evidence-based measures focused on errors, accidents, injuries, and infections.
This achievement marks Montclair Hospital Medical Center’s sixth consecutive ‘A’ Grade, placing the hospital among a select group of facilities recognized for sustained excellence in patient safety and harm prevention.

The Leapfrog Hospital Safety Grade is the nation’s only rating program dedicated solely to preventable medical errors, infections, and injuries – serious events that contribute to more than 500 deaths each day in the United States. The grading system is peer-reviewed, fully transparent, and free to the public. Grades are issued twice annually in the fall and spring.
Montclair Hospital Medical Center maintains its strong safety performance through initiatives such as rigorous infection-prevention practices, enhanced medication-safety protocols, multidisciplinary rounding, and continuous staff education aligned with national best practices.
