Spring 2025 rating honors Montclair’s continued commitment to patient safety and clinical excellence
Montclair, CA, MAY 1, 2025 – Montclair Hospital Medical Center, a not-for-profit member of Prime Healthcare Foundation, has earned an “A” Hospital Safety Grade from The Leapfrog Group, a national nonprofit watchdog that evaluates hospitals on their ability to protect patients from preventable harm. The “A” rating reflects Montclair’s ongoing dedication to delivering safe, high-quality care across all clinical settings.
Montclair Hospital Medical Center has received the “A” Hospital Safety Grade for three consecutive years, placing it among the top-performing hospitals in the region for patient safety. It remains one of the few hospitals in San Bernardino County to consistently achieve this distinction, underscoring its role as a trusted healthcare provider for the Inland Empire.
“Our continued recognition from The Leapfrog Group reflects the hard work and vigilance of every team member at Montclair Hospital,” said Gail Aviado, CEO of Montclair Hospital Medical Center. “Patient safety is at the heart of everything we do, and we are proud to be acknowledged for our success in fostering a culture where safety and quality come first.”
“Achieving an ‘A’ Hospital Safety Grade reflects enormous dedication to patient safety,” said Leah Binder, president and CEO of The Leapfrog Group. “I extend my congratulations to Montclair Hospital Medical Center, its leadership, clinicians, staff and volunteers for creating a culture where patients come first.”
Montclair Hospital Medical Center has also been recognized nationally for excellence in multiple areas of care. These include the American Heart Association’s Get With The Guidelines® – Heart Failure Gold Plus award, the Target: Type 2 Diabetes Honor Roll™, and the 2023 Leapfrog Top General Hospital award—one of the most prestigious designations for hospital safety and quality. The hospital also received the Healthgrades Patient Safety Excellence Award and the Women’s Choice Award® for America’s Best Emergency Care.
As part of its patient safety program, Montclair Hospital has implemented advanced infection prevention protocols, rigorous medication safety checks, and comprehensive staff training programs to ensure that every patient receives the safest and most effective care possible.
The Leapfrog Hospital Safety Grade is the only hospital rating system focused exclusively on preventable medical errors, infections, and injuries that occur in U.S. hospitals—causes that contribute to more than 500 patient deaths each day. The grading methodology is peer-reviewed, transparent, and updated twice annually in the spring and fall.
